Uniform Order Request Form

Order Form Instructions:

Step 1: Click on the View Product Catalog Button to view the scrub tops and bottoms and fleece jacket options.

Step 2: Identify your uniform color using the Care Team chart.

Step 3: Identify two (2) scrub tops and bottoms and one (1) Fleece jacket and utilize the sizing table per style.

Step 4. Write down the style number for each item as they will be needed for the order form.

Step 5: Click on the Return to Order button to return to the order form.

Step 6. Complete all fields in the Uniform Order Form.

Step 7: Click on Place My Order Button

* Important: Carefully review the sizing chart, as sizes may differ by style. Missing order form information may impact the timely processing of your order. No returns on embroidered items.
